Introduction

In accordance with Québec’s Law 25, Dre Cynthia Carbone, Orthodontiste Inc. is committed to ensuring the confidentiality, security and integrity of the personal information it collects through the clinic’s activities, including the use of its website, drcynthiacarbone.com.

Information we collect

On the website, through the appointment request form: your first name, last name, email address, phone number and, if you choose to provide it, the reason for your visit. Please do not include medical details; we will discuss them with you by phone or at the clinic.

The site also uses cookies. Non-essential cookies are only activated with your consent. See our Cookie Policy.

At the clinic, as part of your treatment, through our intake form or digital records sent by your dentist (files or photos):

• your contact information and date of birth;
• your medical and dental history;
• your X-rays, photographs and dental impressions or scans;
• the name of your dentist or other health professionals involved in your care;
• the information needed for billing.

For a patient who is a minor, we also collect the contact information of the parents or guardian.

Why we use it

We use your information only to:

• respond to your requests and confirm your appointments;
• assess, plan, carry out and follow up on your orthodontic treatment;
• communicate with your dentist or other professionals involved in your care;
• bill for our services and confirm fees paid, when your insurer requires it;
• meet our legal and professional obligations.

We never sell your personal information and do not use it for advertising.

Your consent

We collect, use and disclose your information with your consent, obtained through a form provided for that purpose, for the purposes described above, except where the law provides otherwise. For a person under 14, consent is given by the person having parental authority or the tutor. You may withdraw your consent at any time by contacting us, although this may limit the care we can provide.

Who can access it

Only team members who need your information to do their work have access to it. We may also share it, to the extent necessary, with:

• your dentist or other health professionals, for your care;
• an orthodontic laboratory, to make your appliances;
• your insurer, with your authorization;
• our service providers (web hosting, email, practice management software), who are bound by contract to keep it confidential.

We may also disclose information without your consent where the law permits or requires it, for example in response to a court order.

Where your information is stored

At the clinic, your information is kept on paper and in specialized software that encrypts confidential data.

Our website is hosted in Canada. Our email, including messages sent through the website form, is provided by Google, which may store information outside Québec and Canada. If you accept statistics cookies, site traffic data is processed by Jetpack (Automattic) in the United States. Before any such transfer, we assess the protections in place and require adequate protection by contract.

How long we keep it

Messages sent through the website form are deleted once no longer needed or, if you become a patient, added to your file. Patient records are kept for five years, as required by professional regulations, then securely destroyed or anonymized.

How we protect it

We take administrative, technical and physical security measures to protect your information against loss, theft and unauthorized access, including:

• a secure website (HTTPS);
• a firewall and network monitoring software;
• access protected by username and password, limited to authorized staff;
• a confidentiality agreement signed by our staff;
• automated, regular backups;
• secure email;
• paper records kept in a locked area.

Your rights

You may at any time:

• access the information we hold about you;
• have corrected any information that is inaccurate, incomplete or ambiguous;
• withdraw your consent to a use or disclosure;
• ask us to stop disseminating or to de-index information, in the cases provided by law;
• ask that your computerized information be provided to you, or to a person of your choice, in a structured, commonly used technological format.

Some information must be kept to meet our legal and professional obligations, even if you ask for it to be deleted.

Confidentiality incidents

If you believe your information has been accessed, used, disclosed or lost without authorization, please let us know promptly. When an incident presents a risk of serious injury, we notify the people affected and the Commission d’accès à l’information.
